What to expect the first session:
Please expect to arrive at least 15 minutes prior to your scheduled appointment to fill out any additional paperwork.
Please allot at least 75 minutes for your intake assessment, as well as time you need to get to and from your appointment. Follow up sessions may be shorter than this. See 'Services' page for more information on types of services, including information on the initial assessment.

Do you accept insurance:
At this time, the practice is no longer in network with major insurance carriers. I am able to provide a Superbill to submit to your insurance for partial reimbursement upon request. There are tools, such as Reimbursify and Advekit, that can help you submit super bills and receive faster reimbursement when using out of network benefits.
Because the practice is self-pay, under Federal law, you have the right to receive a 'Good Faith Estimate,' explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

All forms for the intake appointment may either be emailed to new clients, or have the option to fill out paperwork on the same day of service prior to the start of your appointment. If filling out paperwork in the office, please plan to arrive at least 15 minutes prior to the appointment as the paperwork may take time to be reviewed and signed. Please feel free to email for a copy of the forms at Counseling@HolenBlackburn.com. Print off all completed forms along with payment for the service. Thank you for your understanding.
